Full Private Health Care
If your annual gross income of the last three consecutive years exceeded a limit of € 49.950, you have the right to opt out of the state public health system and insure yourself with a private health insurer.
The main differences between public and private health insurance in Germany are:
The State Health System
- Families can be insured as one unit. The breadwinner pays a health subscription based upon his/her income and all other non- employed family members are automatically covered for free
- Working women pay no subscriptions to the State System six weeks prior to and eight weeks after the birth of a child
- Income based subscriptions; the more you earn, the more you pay
- Standard health programs dictated by those operating the system
- Treatment by private doctors or consultants often have to be paid by yourself
- Only essential medical treatments covered
- Certain prescribed medicines and treatments excluded
- Alternative and psychotherapeutic medicine excluded
- Only ward accommodation in hospitals and no choice in which hospital treatment is given
- Restricted dental treatment
The Private Health System
- Monthly premia based upon the coverage chosen, sex, age and medical history of insured; not related to income
- Each family member must be insured separately
- Doctors appointments generally available on shorter notice than in the public system
- Generally shorter waiting periods for hospital treatment
- Coverage for virtually all treatments and prescribed medicines possible ( check specific tariff descriptions for details )
- Insured may be treated by any doctor, dentist, Consultant or specialist
- Alternative medical treatments and psychotherapeutic medicine covered
- Free choice of hospitals with treatment by a senior Consultant or professor and accommodation in a single or double room possible
- Health programs tailored to an individual's medical needs and chosen by the insured
- Financial rewards for low medical costs
Generally speaking, if you earn enough to be privately insured then you will often pay substantially less for your health care if you opt out of the State System and choose a private health plan.

Supplementary Private Health Care
If you are in the public/ state health system it is possible to purchase supplementary private health insurance. Such insurance can be purchased for all medical treatment and can entitle members of the state health system to:
- Hospital accommodation in a single/double room and treatment by a Consultant or professor
- Extended dental & optical
- Extended out patient care (GPs, ob-gyn, etc.)
- Please be aware that the supplementary insurance will not shorten waiting periods with a doctor or a hospital as the full private coverage most probably will
